Statement of Fitness to Work (Fit note)

A ‘fit note’ can be provided by a GP or other clinician who has assessed your case and found that you are not fit for work or may only be fit for limited duties (specified on the certificate).

The NHS provides sickness certification only after seven continuous days of illness.  If you need this for 7 days or less you should complete a Self Certification (SC2) form which is available from your employer or on the HMRC website.

You do not need a note to say you are fit to return to work.

A new version of the fit note was introduced in April 2022 which no longer requires the issuer to sign the form.  Please visit this government webpage for more information about changes to the fit note.
