About Our Online Services

By signing up for online services, you will be able to complete a variety of different tasks online rather than having to come in to the practice:

  • View past and future appointments
  • Order recent and regular repeat medications
  • View parts of your medical record such as vaccine history (including Covid vaccines)
  • Access test results

These online services are as safe as online banking as long as you keep your passwords secure, and you can access these services 24 hours a day 7 days a week.

How to Sign Up

You will need to have a username and password created for you, before you can use our online services.

In order to sign up for online services, simply come into the practice to fill in a registration form for online services, or download the SH Patient Access Application Form and bring into the surgery with some form of ID.

 Forms of identification accepted are:

  • Passport and/or Birth Certificate
  • HC2 certificate
  • Mortgage Agreement
  • Utility Bill
  • Driving Licence
  • Rough sleepers’ identity badge
  • Hostel registration/mail forwarding letter

Proxy Access

You can also sign up for online access on behalf of another patient, ie a child or partner. This is know as Proxy online access.

You will be able to access their record through the same log in you use for your own online account, as a linked patient. It is advisable to have online access for yourself before registering as a proxy user.

To request proxy access you must complete a Proxy access form. On page 1 complete the details for the patient you want access for. Children under 11 do not need to sign the form. If the patient is aged over 16 they must sign the form and show ID when registering.

On page 2 complete the details for the proxy user, ie. Mum or Dad. Each person who wants access must complete all their details, sign the form and show ID when registering.

If the form is not completed correctly we will not be able to complete the registration for proxy access.
